The incident investigation training will describe the basic approach to responding to an incident, covering all phases of the investigation from immediate actions, data gathering, execution of the investigation and most importantly developing effective corrective and preventive actions.

Compared to most courses that cover incident investigation, this incident investigation training has the distinct advantage of being provided by a world leading incident investigator, who developed his skills during the Piper A incident, which is the worst accident to occur in the offshore oil/gas industry and led to significant improvement is HSE within the oil/gas sector.   • Developing world class incident investigation processes applicable to any occupational, major hazards and process safety
    incidents.
  • Drafting the Terms of Reference for incident investigations including defining the boundaries of the investigation and the critical aspects of data collection, analysis, statement taking and reporting.
  • Conducting investigations and the tools and techniques for interviewing witnesses and confirming with evidence the accuracy of the statement.
  • Production of investigation reports that establish key aspects of the event such as: the incident timeline, the causation models such as root cause and why-why analysis and the linkage between causes, recommendations and actions
  • By taking part in real accident case studies – onshore and offshore – being able to define preventive and corrective actions and distinguishing action effectiveness based on proven concepts.
  • Developing Emergency and Crisis Management Response Plans and the roles and responsibilities of key response personnel.
  • Understanding of risk control barriers and the importance of human factors and organisational competency for their effect on incident causation factors.
