SUMMARY:HAZOP Training: Hazard Studies and HAZOP Team Leader
DESCRIPTION:Why Choose this Training Course\nThis is a 3-day HAZOP training course. HAZOP and Hazard studies are an essential part of effective process safety management and HSE risk management. This course covers various process hazard assessment (PHA) techniques that team leaders must understand and be able to apply as well as facilitation skills required to manage successful hazard study meetings.\n \nThese techniques apply to all industry sectors including oil & gas\, petrochemicals\, chemicals\, pharmaceuticals\, and utilities\, both on new projects and on existing operating plants. Each hazard study technique aligns with a project stage and the needs of the level of project development at that stage.\n \nThe course consists of a series of modules that deal with the different hazard study stages and techniques and related process safety concepts and facilitation topics. Presentations are interactive with video illustrations and case studies. Group exercises are used for the different techniques (including\, inherent safety\, HAZID\, HAZOP and procedural HAZOP)\, to enable attendees to put into practice what they have learnt during the lecture presentations.\n \nThe HAZOP training course is intended to be an open course using generic (non-proprietary) examples and case studies but can be adapted with site specific case studies if available. \nWho Should Attend\nThe HAZOP training course is designed for participants who are expected to lead HAZOPs and already have some experience as HAZOP team members. It is also appropriate for regular team members who wish to develop their HAZOP and PHA skills further by hazard. \nTypical attendees would be: \n\nProcess Safety Engineers\nTechnical Safety Engineers\nProcess Design Engineers\nLoss Prevention Engineers\nTechnical Safety Managers\nProcess Safety Managers\nProcess Safety Advisers and Specialist\nTechnical Safety Advisers and Specialist\nTechnical Safety Authorities\nSafety/HSE Managers\nProcess Safety Consultants\n\nEnquiry Form\n\n\n \n \n SalutationMr.Ms.Miss.Mrs.Mdm.Dr.Prof.Capt.-Name*\n \n \n \n First\n \n \n \n \n Last\n \n \n Phone*Email*\n \n Company*Current Location* \n \n \n AfghanistanAlbaniaAlgeriaAmerican SamoaAndorraAngolaAnguillaAntarcticaAntigua and BarbudaArgentinaArmeniaArubaAustraliaAustriaAzerbaijanBahamasBahrainBangladeshBarbadosBelarusBelgiumBelizeBeninBermudaBhutanBoliviaBonaire\, Sint Eustatius and SabaBosnia and HerzegovinaBotswanaBouvet IslandBrazilBritish Indian Ocean TerritoryBrunei DarussalamBulgariaBurkina FasoBurundiCambodiaCameroonCanadaCape VerdeCayman IslandsCentral African RepublicChadChileChinaChristmas IslandCocos IslandsColombiaComorosCongoCongo\, Democratic Republic of theCook IslandsCosta RicaCroatiaCubaCuraçaoCyprusCzechiaCôte d'IvoireDenmarkDjiboutiDominicaDominican RepublicEcuadorEgyptEl SalvadorEquatorial GuineaEritreaEstoniaEswatiniEthiopiaFalkland IslandsFaroe IslandsFijiFinlandFranceFrench GuianaFrench PolynesiaFrench Southern TerritoriesGabonGambiaGeorgiaGermanyGhanaGibraltarGreeceGreenlandGrenadaGuadeloupeGuamGuatemalaGuernseyGuineaGuinea-BissauGuyanaHaitiHeard Island and McDonald IslandsHoly SeeHondurasHong KongHungaryIcelandIndiaIndonesiaIranIraqIrelandIsle of ManIsraelItalyJamaicaJapanJerseyJordanKazakhstanKenyaKiribatiKorea\, Democratic People's Republic ofKorea\, Republic ofKuwaitKyrgyzstanLao People's Democratic RepublicLatviaLebanonLesothoLiberiaLibyaLiechtensteinLithuaniaLuxembourgMacaoMadagascarMalawiMalaysiaMaldivesMaliMaltaMarshall IslandsMartiniqueMauritaniaMauritiusMayotteMexicoMicronesiaMoldovaMonacoMongoliaMontenegroMontserratMoroccoMozambiqueMyanmarNamibiaNauruNepalNetherlandsNew CaledoniaNew ZealandNicaraguaNigerNigeriaNiueNorfolk IslandNorth MacedoniaNorthern Mariana IslandsNorwayOmanPakistanPalauPalestine\, State ofPanamaPapua New GuineaParaguayPeruPhilippinesPitcairnPolandPortugalPuerto RicoQatarRomaniaRussian FederationRwandaRéunionSaint BarthélemySaint Helena\, Ascension and Tristan da CunhaSaint Kitts and NevisSaint LuciaSaint MartinSaint Pierre and MiquelonSaint Vincent and the GrenadinesSamoaSan MarinoSao Tome and PrincipeSaudi ArabiaSenegalSerbiaSeychellesSierra LeoneSingaporeSint MaartenSlovakiaSloveniaSolomon IslandsSomaliaSouth AfricaSouth Georgia and the South Sandwich IslandsSouth SudanSpainSri LankaSudanSurinameSvalbard and Jan MayenSwedenSwitzerlandSyria Arab RepublicTaiwanTajikistanTanzania\, the United Republic ofThailandTimor-LesteTogoTokelauTongaTrinidad and TobagoTunisiaTurkmenistanTurks and Caicos IslandsTuvaluTürkiyeUS Minor Outlying IslandsUgandaUkraineUnited Arab EmiratesUnited KingdomUnited StatesUruguayUzbekistanVanuatuVenezuelaViet NamVirgin Islands\, BritishVirgin Islands\, U.S.Wallis and FutunaWestern SaharaYemenZambiaZimbabweÅland Islands\n Country\n \n \n How many people are you looking at?1 - 2 Standard Rates3 - 5 Group Discount>10 In-House Training\, Huge Savings!Undecided\, need more informationThis is just an approximate number. SUMMARY:Best Practices in the Management and Execution of Incident Investigations
DESCRIPTION:Why Choose this Training Course\nMajor accidents occur infrequently\, but can result in consequences beyond what the management envisage\, resulting in a situation that escalates beyond the capacity of the company to recover both financially and reputationally. The 2010 Deepwater well blow-out accident is an example\, whereby the company\, BP\, had to deal with a significant financial penalty associated\nwith the environmental impact as well as the personnel casualties and reputational damage. During this three-day course\, participants will discover the real challenges that face the Executive Management team and their sub-ordinate managers and staff managing any project or operation within a global business developing and operating high hazard facilities in the oil\, gas\, chemicals\, nuclear and all other hazardous facilities. \nThe incident investigation training will describe the basic approach to responding to an incident\, covering all phases of the investigation from immediate actions\, data gathering\, execution of the investigation and most importantly developing effective corrective and preventive actions. \nCompared to most courses that cover incident investigation\, this incident investigation training has the distinct advantage of being provided by a world leading incident investigator\, who developed his skills during the Piper A incident\, which is the worst accident to occur in the offshore oil/gas industry and led to significant improvement is HSE within the oil/gas sector. Importantly\, the lessons learned during that investigation provided a benchmark for best practices in incident investigation. \nOn completion of the incident investigation training\, attendees should be capable of: \n\nDeveloping world class incident investigation processes applicable to any occupational\, major hazards and process safety\nincidents.\nDrafting the Terms of Reference for incident investigations including defining the boundaries of the investigation and the critical aspects of data collection\, analysis\, statement taking and reporting.\nConducting investigations and the tools and techniques for interviewing witnesses and confirming with evidence the accuracy of the statement.\nProduction of investigation reports that establish key aspects of the event such as: the incident timeline\, the causation models such as root cause and why-why analysis and the linkage between causes\, recommendations and actions\nBy taking part in real accident case studies – onshore and offshore – being able to define preventive and corrective actions and distinguishing action effectiveness based on proven concepts.\nDeveloping Emergency and Crisis Management Response Plans and the roles and responsibilities of key response personnel.\nUnderstanding of risk control barriers and the importance of human factors and organisational competency for their effect on incident causation factors.\n\nWho Should Attend\nThe course is aimed at people who wish to attend the course and put into practice the key tools of managing human factors in their company. This may include: \n\nHuman Error Investigation Specialists\nHuman Factors Design Engineers\nCompetency and Training Managers\n\nIn addition\, those involved in the design / operation of their facilities will also benefit from the course\, such as: \n\nHSE Coordinators\nHSE Engineers\nHSE Managers\nMaintenance Managers\nManagers responsible for the safe operation of facilities\nMechanical\, Process\, Instrument and Control Engineers\nProcess Safety Managers\nProject Engineers\nReliability Engineer\n\nKey Learning Objectives\n\nReview a range of best practices used by global companies for the development of occupational and major incident investigation processes.\\nGain in-depth knowledge from a recognized expert in investigating major accidents\, including incident reporting\, data gathering\, taking witness statements and the application of incident causation analysis.\nUnderstand how to structure effective corrective and preventive actions arising from an incident investigation\nGain an appreciation of behavioral safety and human factors should be incorporated within the investigation\nLearn about the impacts on the business arising from incidents\, including how to categorise actual and potential\nBuild knowledge of how executive teams and their line/functional managers control HSE risks in the business and how\nHSE managers conduct annual ISO management reviews\, crisis and emergency response training and the definition and reporting of Key Performance Indicators.\nGain knowledge of technical safety\, bow-tie and barrier management and how proactive safety management is used to determine barrier effectiveness and hence identify potential incident causation factors\nObtain an appreciation of the key issues faced when developing the investigation\, writing terms of reference and assigning\nthe investigation team HSE legislation – particularly defining risk management requirements.\n\nEnquiry Form\n\n \n \n SUMMARY:TUV PHRA Training: TÜV Rheinland FS Engineer Process Hazard and Risk Analysis
DESCRIPTION:Why Choose this Training Course\nThis TUV PHRA training course focuses on the early lifecycle phases of hazard and risk analysis up to\, and including\, the development of the Safety Requirements Specification (SRS) for process\, oil & gas\, petrochemical and chemical industries according to IEC 61508 / IEC 61511. \nThis 5-day course comprises 4 days of classroom tuition and practical guidance\, for understanding and mastering the application\, of process hazard analysis and associated risk analysis as required for the early lifecycle phases of Edition 2 IEC 61508 & Edition 2 of IEC 61511 for SIS. We also include many fundamental topics that we know you need to understand\, but are not covered in the standards. Practical exercises will be performed throughout the course based on real-life examples. There is a 5-hour competency examination on the last day. \nWho Should Attend\nProcess Engineers\, Instrument Engineers\, Process Safety and Operations personnel who are involved in maintaining the integrity of their processes\, and are more involved in the early lifecycle phases of functional safety management. \nKey Learning Objectives\nThe topics cover the concepts of identifying hazards using tools such as What-If\, Failure Mode and Effect Analysis (FMEA) and HAZard and OPerability (HAZOP). This will be further developed into the analysis of risk and methods of risk reduction to consider the effectiveness of preventative and mitigating layers of protection. The use of preventative safeguarding efficiencies will be introduced for use with the PHA Matrix\, along with the advantages/disadvantages of using the PHA Matrix with different Hazard Analysis methods. \nThe different questioning techniques used by a Hazard study leader to guarantee a systematic study methodology are investigated\, as is the HAZOP flowsheet design\, by considering the use of the HAZOP for specific needs such as LOPA. A LOPA exercise will actually be carried out from the output of the course HAZOP exercise. Consideration of constructing effective nodes will be examined and a better definition will be given to claims of double jeopardy. \nThe TUV PHRA training will introduce participants to the use of Safety Integrity Levels (SIL) and SIL determination using internationally agreed methods for assessing and quantifying process risk and risk reduction requirements. Delegates will be equipped with the knowledge and methods for undertaking various types of qualitative\, semi-quantified and quantitative risk assessments. \nPractical exercises will be carried out for Semi-Quantitative methods such as risk graph and LOPA. The shortfalls of the quicker methods will be investigated to allow the analyst to appreciate the limitations of screening tools. The most popular semi quantified method\, Layer of Protection Analysis (LOPA)\, will be explored in depth. \nThe effective use of Event Tree Analysis will be looked at in two different forms (Mitigation ETA and Traditional ETA). The TUV PHRA training will focus on how ETA can be used to qualitatively support the analyst in mapping out the various consequences given various events with safeguard success or failure. Analysts can then select those events which generate consequences of interest for which FTA may be required. \nDelegates will also be taken through the requirements for safety management and the framework of a Safety Management System (SMS) for achieving effective process risk management. Best practice for SMS will be investigated with the use of Bow-ties linking the Hazard and its safeguards to the SMS. An insight into how process safety KPIs can be used to better align engineering and management in the prevention of major accident hazards will be covered. \nPartnership\n\n\n\nTUV Rheinland Singapore was founded in 1989 and has been serving the testing and certification needs of Singapore companies for over 20 years.
